I understand that all instructors and students participate at their own risk, and I unreservedly accept this as a condition of my training, recognising that Brazilian Jiu-Jitsu (BJJ) can be hazardous. I, the undersigned, hereby voluntarily submit this 'Application to Train' at One Jiu-Jitsu in order to partake in martial arts. I am fully aware of the nature, inherent dangers, and risks involved in this activity. I assume full and complete responsibility for any and all losses, damages, or injuries that I may sustain or incur while attending, participating in, or travelling to and from any events or sessions arranged by One Jiu-Jitsu, its clubs, or affiliates. If I am signing on behalf of a child or minor, I accept that all terms and conditions of this agreement apply equally to said child or minor. I hereby waive all claims against One Jiu-Jitsu, its instructors, assistants, sponsors, and any institutions or organisations connected with or associated with One Jiu-Jitsu, for any and all injuries that I may sustain. By signing below, I certify that I fully understand the contents of this Release Statement and fully agree to the Terms and Conditions herein stipulated. This agreement is executed in accordance with the laws of the United Kingdom and is intended to be legally binding.

I consent to the data provided to be used for the purposes of registration within the dojo and to provide emergency contact details for use when I am training.

CCTV Recording - For security purposes, I acknowledge that One Jiu-Jitsu has CCTV cameras recording 24/7 within its facilities. These recordings are used solely for the safety and security of the clients, staff, and property. I consent to being recorded while on the premises.

I understand that One Jiu-Jitsu frequently records videos and takes photographs for social media promotions and marketing purposes. I consent to the use of such recordings on One Jiu-Jitsu’s platforms unless I explicitly notify the staff in writing that I do not wish to be included. By signing this waiver, I grant permission to One Jiu-Jitsu to use my image and likeness in promotional materials. If I am signing this waiver as a parent or guardian of a minor participant, I also give permission for One Jiu-Jitsu to use my child's image and likeness in promotional materials unless I explicitly notify the staff in writing that I do not wish for my child's image to be included.
